This "Notice of Revocation of Authority" is used by an employer (the “Authorizing Person”) who wishes to withdraw an authorization previously granted to an employee (the “Revoked Employee”) to act as legal representative of the employer.
As result, the Revoked Employee shall cease to act as a representative of the employer’s organization from the date of the receiving the revocation.

This Notice of Revocation of Authority must be signed by the Authorizing Person and must be countersigned by the Revoked Employee as a proof that the latter has actual knowledge of this revocation. It should be noted that this notice shall be sent by registered mail/hand/email or by any other appropriate method that allows the Authorizing Person to proof that the revoked employee has been notified of his/her revocation. |
